Bodywork Training RegistrationBooking ProcedureYour completed form can be sent electronically, or printed and sent by post. Last minute bookers may be disappointed and cheques must arrive in time to be cleared. Kindly include your full address and telephone number(s). If you wish to have your place confirmed or fee receipted please enclose a S.A.E. The balance of the fee for a 2-day group is payable, on arrival at the group, in cash or by cheque supported by bankers card. If you know someone else who would be interested, please give their name and address so she or he can receive a leaflet through the post. Registration FormI agree to pay the balance of the fee, as and when due. Cheques should be made payable to Bodyspace and posted to Bodyspace, P.O. Box No. 28430, Upper Holloway, London N19 3ZN. Kindly note that deposits are non-refundable and only transferable at my discretion. For any non attendance or withdrawal notified less than 7 days before a weekend the full fee for that weekend remains payable.
